Stilson On Drill Mast Incident

In addition to the below incident, there are numerous other similar incidents and injury reports that go back a number of years.

What is frustrating, from an industry perspective, is that we are still having these types of incidents on rigs BUT, if we stop using stilsons on drill masts, (and live drill strings)  we will eliminate these incidents.

On 20 June 2024, at 21:15pm on the OSR 4740 bench, Drill Rig 122 was tasked with drilling boreholes for Geotech and instrumentation. The drilling crew was installing surface casing to a target depthof 20ft in poor ground conditions. While installing the casing, the threads on the casing became overtorqued and could not be broken apart. The crew attempted to back up a 36-inch pipe wrench to drillmast in order to break the threads apart when the wrench slipped, rotated around and impacted thedrill helper’s lower legs resulting in a fracture to one leg. Mayday procedures were initiated, ERT wascalled, and the IP was assessed then was transported to the hospital for further assessment andtreatment.
